INSTRUCTIONS FOR GRANT-IN-AID APPLICATION

The application is filled out online. After submitting the application information and additional required documents, a copy of the application and signature page will be prepared. These can be downloaded.

After downloading the application, please print out 8 copies of the completed application.

For the signature page:

  • Please print out one copy of the signature page and have it signed
  • Please make 8 photocopies of the signed signature page
  • Please send a scan to email of the signature page to director@stonywoldherbertfund.org

Please send the following via Express Mail:

  • The 8 hard copies of the application
  • The original signature page containing original signatures
  • The 8 photocopies of the signature page

GENERAL INSTRUCTIONS

The Stony Wold-Herbert Fund will support grants-in-aid for research, both clinical and basic, in the field of tuberculosis and allied chest diseases. The major emphasis of its research program is the support of investigators of outstanding ability through the support of their research work.

The desirability of grants-in-aid will be judged primarily by the likelihood of fruitful accomplishment as indicated by:

 

A.  The responsible investigator.

B.  The research climate of the institution where the work will be done.

C.  The merits of the research project.

Grants are awarded by the Board of Directors on the recommendations of the Medical Advisory Committee and are available to non-profit institutions which, in the opinion of the Committee, provide the necessary staff and research facilities to support the responsible investigators.

Consideration will be given to proposed research in the field of tuberculosis, allied chest diseases, and to related pulmonary ailments. Support is available for all basic disciplines of physiology, chemistry, and pathology which relate to respiratory diseases, as well as for clinical investigations.

No application will be considered unless completely executed, including signatures of Department Head and authorized institutional official.

Grants-in-aid will be awarded only to individuals working in institutions identified as Equal Opportunity Employers.

PROGRESS REPORTS AND PUBLICATIONS

Six-month and final reports are required, and the final report should also include full budget accounting.

It is expected that publications based on any study or research done during the period of grant-in-aid support will acknowledge the award and that a reprint is sent to the Stony Wold-Herbert Fund.

PROGRESS REPORTS DUE

  • December 15th - Six-month Report
  • June 30th - Final Report

NOTIFICATIONS

The Fund must be notified immediately if you are awarded any duplicate or overlapping funding, if you encounter any difficulties completing this grant, or any change in your status. A full report including budget accounting must be submitted upon completing of this grant.

FINANCIAL SUPPORT

As determined by the nature of the research project, the sum granted may cover the costs of supplies, animals and board, a limited amount of special equipment, essential travel and publication expenses and other necessary expenses, as well as the salaries for technical personnel. Grants requesting salaries for the principal investigators are not considered.

A listing of any other funds available for this work or to the investigating team must be given. If none, so state.

Proposed budgets should not include miscellaneous, contingent or overhead expenses. The Fund makes no provision for institutional overhead expenses.

Awards are made on an annual basis and paid semiannually on July 1st and December 15th directly to the Institution.

DURATION

Grants are awarded for one year but may be renewed by reapplication and on submission of a satisfactory progress report. The responsible investigator will furnish an annual report containing a concise statement of the progress achieved under the grant.

RESPONSIBLE INVESTIGATOR

The responsible investigator will be on the regular staff at the institutions where the work will be conducted.

EQUIPMENT

When the grant terminates, equipment purchased with grant funds becomes the property of the institution where the research was conducted.

PUBLICATIONS

It is required that discoveries resulting from research supported entirely, or in part, by the Fund be made fully and promptly available to the public through approved scientific channels. The Stony Wold-Herbert Fund must be acknowledged.

PATENTS

Discoveries from research carried out under a grant-in-aid will be subject to the current patent policies of the Fund. Written approval to apply for the patents must be obtained from the Board of Directors of the Stony Wold-Herbert Fund following recommendations by the Medical Advisory Committee and by the patent policy administrator of the institution where the research work was done.

OUTLINE FOR APPLICATION FOR RESEARCH GRANT-IN-AID

  1. A concise outline of the proposed research, to include a statement of:
    • The aims of the proposed research program.
    • Previous work done on the same or related program by the principal investigator and other investigators.
    • Research methods to be used on the program.
    • The significance of the research.
  2. Bibliography: Provide significant references to work related to proposed research by the responsible investigator or other investigators.
  3. Curriculum Vitae of applicant and investigators: A curriculum vitae for the investigator responsible and all collaborating and professional investigators is required. The curriculum vitae should be chronologically arranged and include: date and place of birth, education with names and addresses of secondary schools, colleges and universities with dates of attendance, degrees and dates obtained; societies – honorary, scientific and scholastic; military service with dates rank and location.
  4. Professional positions held by investigators, previous and current with titles, institutions and dates.
  5. Budget: (See section on financial support of General Instructions).
  6. Renewal applications: Renewal applications for the continuation of projects beyond one year are considered only when required and expenditure reports have been submitted.

Applications must be received by October 15th.

Action will be taken on the application before January 1st. The applicant will be notified  and, except under unusual circumstances, the term of the grant will be from July 1st to June 30th
