INSTRUCTIONS FOR TRAINING FELLOWSHIP APPLICATION

The application is filled out online. After submitting the application and additional required forms, three documents will be available to download:

• The application

• The Signature Page

• Instructions for preparing The Sponsor’s Statement

Please download and print out 8 copies of the completed application

Please download and then print out 1 copy of the signature page, and have it signed. Please make 8 photocopies of the signed signature page.

Please give the instructions for preparing The Sponsor’s Statement to your sponsor.

Please send via Express Mail:

• The 8 hard copies of the application

DATES:    Applications, sponsors' endorsements and letters of reference must be received no later than October 15^th  to be considered for commencing the following July. Applicants are advised they may be interviewed.

It is the responsibility of the applicant to request the sponsor's endorsement and letters of reference and to make certain they reach us before the October 15^th deadline.

Action will be taken on the application before January 1^st. The applicant will be notified  and, except under unusual circumstances, the term of the grant will be from July 1^st to June 30^th. 

OUTLINE FOR APPLICATION FOR TRAINING FELLOWSHIPS

The Stony Wold-Herbert Fund, Inc., offers fellowship awards for training in the field of respiratory diseases, including tuberculosis.

The objective of the Fellowship program is to stimulate the training of teachers, scientific investigators and clinicians in the field of lung disease in the greater New York City area.

 

 

QUALIFICATIONS

Applicants may be:

a.     Physicians entering the third postgraduate year or late year of training in Internal Medicine, Pediatrics, Thoracic Surgery or other specialties, including basic sciences relevant to lung disease, who are interested in a career of teaching, investigation or practice in the field of lung disease. Priority will be given to applicants interested in an academic career.

b.     Holders of other doctoral degrees (e.g. Ph.D.) interested in further training as scientific investigators in the field of prevention and control of lung disease.

c.     In special circumstances, other applicants with comparable qualifications for research related to the lung may be considered.

d.  Each applicant is expected to have already obtained an appointment in a university, medical center or hospital for training under a responsible investigator.

AWARDS

Fellowships will be awarded only to individuals working in institutions identified as Equal Opportunity Employers.    

Awards are made on an annual basis and paid bi-annually on July 1^st  and December 15^th directly to the Institution. An annual report of expenditures must be furnished to the Fund by the responsible financial officer. 

TERMS OF FELLOWSHIP

Fellowships are awarded for one year. Applications for renewal must be submitted by October 15^th of the first year of funding. First-time renewals are usually approved by the Fellowship committee and the Board of Directors. Applications for the third year of funding are generally not approved and will be given a low priority.

No other fellowship may be held concurrently with the Training Fellowship, nor may the Fellow engage in the private practice of medicine during the award period.

Specific services are not required of the Fellow by the Stony Wold-Herbert Fund.

PROGRESS REPORTS AND PUBLICATIONS

The final report should also include full budget accounting.

It is expected that publications based on any study or research done during the period of fellowship support will acknowledge the award and that a reprint is sent to the Stony Wold-Herbert Fund.

In the case of a renewal application, a letter from the program director and a brief progress report for the first three months must be submitted by the applicant.

NOTIFICATIONS

The Fund must be notified immediately if you are awarded any duplicate or overlapping funding, if you encounter any difficulties completing this grant, or any change in your status.
