INSTRUCTIONS FOR COMMUNITY SERVICE GRANT APPLICATION

SUMMARY

The Community Service Grants Program helps to finance and encourage the development of ongoing community-wide projects serving large numbers of pulmonary patients. The Stony Wold - Herbert Fund awards a total of $40,000 to community-oriented programs.

 

 

OBJECTIVES

While the main objective of this program is to assist the community in its struggle against pulmonary disease, the most effective approach to grant making and grant seeking is to be extremely focused. With so much work to be done, everyone in the world of charitable work should be aware of exactly how every dollar will be put to use. As a result, the Fund offers this list of primary objectives:

A.  To stem the rise of tuberculosis among the poor and the homeless. Recent data indicate that the living conditions for the very poor and the homeless have led to an increase in tuberculosis. The Stony Wold – Herbert Fund will provide funds for projects that either directly help alleviate the situation or gather research and statistical data toward this goal.

B.  To relieve the stress and improve the psychological outlook of chest patients requiring long-term hospitalization. Experience has shown that recreational activities and art therapy treatment have helped long-term chest patients cope with their confinement and possibly contribute to a quicker recovery.

C.  To support Health Education and Referral Programs for matters of respiratory health. Many conditions are left untreated because people do not know what to do or where to find help. Funding in this area will go to organizations which fill gaps in community education and referral services. Special emphasis is placed on services which facilitate early disease detection or intervention to help prevent a potential respiratory problem from escalating into a crisis for the patient.

ELIGIBILITY AND EXCLUSIONS

In order to qualify for a Community Service Grant, the applicant must:

  • be a tax exempt organization or seeking funds for a project that would qualify as tax exempt.
  • must also be involved in work that fits generally into the Fund’s guidelines.

With regard to exclusions, the Fund does not let these grants overlap with other programs it sponsors. Also, since the size of these grants is limited, the Fund is reluctant to participate in proposals where the contribution would have minimal impact. As a result, the Fund does not award Community Service Grants for:

  • Grants outside the New York metropolitan area.
  • Scholarships.
  • Medical Research Grants.
  • Grants to Individuals.

Other low priority items include:

  • Endowments.
  • Building campaigns.
  • Deficit financing.
  • General operating support or annual giving.
  • Religious purposes.
  • Capital campaigns.

OTHER INFORMATION

RENEWAL POLICY: The Fund’s objective is to provide one-time financing to a worthwhile project which will then be able to carry on with financing obtained from other sources. The Fund, however, will consider a proposal for a different project from the same organization at any time (but will not fund two projects simultaneously). The Fund will also consider re-funding an ongoing project, if at least three years have elapsed since the last grant was awarded. A complete application will again be required.

CONFERENCES: Personal interviews and on site visits will be arranged as deemed appropriate, but only after the written proposal has been studied.

REFERENCE CHECKS: Part of our review process may include contacting references as stated in the proposal; therefore the Fund cannot guarantee confidentiality.

DEADLINES AND NOTIFICATION: The detailed proposal is due by March 1st. The Board of Directors will decide which projects will be funded at the May Board meeting. Notification will be made shortly thereafter.

Please remember that the review committee has heavy demands on its time and, although the deadline is March 1st, it is in everyone’s interest to submit your proposal as early as possible.

TERMS OF GRANT

Shortly after winning approval, successful applicants will receive a grant award letter which outlines the terms and conditions of the award. All grantees are expected to comply with these terms and conditions.

Because of budget limitations, the Fund cannot give grants for all the worthwhile proposals submitted and generally cannot provide funds for the entire projects. It is therefore important to solicit other organizations simultaneously for resources to work in concert with the funding you may be awarded by this organization.

REPORTING REQUIREMENT

As a requirement for receiving funding from the Stony – Wold Herbert Fund, grantees are expected to submit a six-month progress report and a year-end program evaluation. These reports should contain financial and narrative data. The year-end evaluation should also address whether or not the program was a success, why or why not and whatever quantitative results are available.




Applications will need to be prepared and submitted in the following manner:

  • Statement of whether this is a Renewal or not: Type “Renewal” or “No”
  • Organization Name

Institution’s Financial Officer information, including:
  • Name and title
  • Financial officer’s email

  • Address where checks should be sent

PART B - A 2-Page Summary:
Submissions of the Application information below that are longer than two pages cannot be accepted.
Using no more than two pages of   1 1/2 spaced typing    please submit the following information:

  • Specific purpose of the grant
  • Anticipated end results
  • The need or problem that the program addresses
  • How the proposed program will address the need/problem stated above, including a specific statement of the program’s objectives
  • If possible, a brief discussion of similar programs and their performance
  • Proposed methodology
  • A statement of which specific population groups will benefit, include any pertinent geographic and demographic information
  • Timetable for the program, including the most desirable start and termination dates for a grant
  • Why your organization is uniquely suited to initiate this program
  • A statement of how and when the success of the program will be evaluated
  • A detailed budget
  • Names of other funding sources contacted
  • Plans for future financial support, assuming the program is of an ongoing nature. Please be very specific
  • A brief statement on the background of your organization
